Emergency Admissions ....

Exalon Care will consider emergency admissions in extreme circumstances.

Our strict criteria require a current Community Care Assessment, Care Plan and any other relevant documentation to be made available to us before the admission decision is taken. We expect them to be provided by the Care Manager/Social worker, potential service user or previous placement as applicable. If we can meet the specific needs of the potential service user, written confirmation will be sent to the service user and/or the appropriate representatives offering a placement under a 3 month trial period.

Here Is how We Will Deliver & Manage Emergency Admissions

  • Locally based
  • Our staff have flexible contracts and can therefore accommodate additional hours to meet emergency admissions.
  • Both homes provide 24 hour care.
  • Both homes have identified respite bedrooms enabling a full assessment to be carried out on further placement.
  • The Willows - the emergency accommodation is a large room situated on the ground floor with an en-suite shower room.
  • The Oaks - the emergency accommodation is a large room based within The Ground Floor Apartment.
  • There is emergency on-call to senior care staff and management.
  • Both services have a “Buddy System” to assist the individual during their stay.
  • For any emergency admission we will conduct a comprehensive Care and Support Plan on admission to ensure the support staff are able to meet the service users' assessed needs.
